In order to be reimbursed for our medical massage treatments, Insurance companies require a Prescription Form filled out by a Medical Doctor (MD), Chiropractic Doctor (DC), Osteopathic Doctor (DO), or Acupuncturist.
We have such forms in our office and we can fill them out on your behalf and you can just have your physician or health care practitioner sign the form for you.
